What are spider veins, and why do they appear?

Spider veins (telangiectasias) are small, dilated blood vessels (venules or capillaries) located very close to the skin’s surface. They appear as thin red, blue, or purple lines or in the form of “spider veins.”

Although they are usually just a cosmetic issue, varicose veins in the lower extremities may be associated with venous insufficiency and accompanied by symptoms such as a feeling of heaviness, burning, itching, or localized pain, especially after prolonged standing.

 

What factors cause spider veins to appear?

The appearance of spider veins is due to the gradual weakening of the walls of the blood vessels and their valves. The main contributing factors include:

  • Genetic Predisposition & Heredity: This is the strongest risk factor, especially in the lower extremities.
  • Hormonal Fluctuations: Pregnancy, birth control use, hormone replacement therapy, and menopause affect the relaxation of blood vessel walls.
  • Lifestyle & Occupation: Prolonged standing or a sedentary lifestyle hinder the proper return of blood to the heart.
  • Sunlight & Heat: Unprotected sun exposure, frequent use of saunas, hot baths, or hot wax for hair removal can worsen spider veins, especially on the face.
  • Age & Excess Weight: Over time, blood vessels lose their elasticity, while excess weight puts additional pressure on the venous system.

 

In which areas do they occur most frequently?

Spider veins are found mainly in two areas of the body:

  • Lower Extremities (Legs): They appear on the thighs, calves, ankles, and the inner surface of the knees, often due to increased venous pressure.
  • Face: They appear mainly on the sides of the nose, the cheeks, and the chin (rosacea, photoaging).

 

How can spider veins be prevented?

Although genetic predisposition cannot be changed, certain habits can delay the onset or progression of these conditions:

  • Regular physical activity (walking, swimming) to improve blood circulation.
  • Avoid standing or sitting for long periods of time (take short breaks to walk around).
  • Maintaining a healthy body weight.
  • Avoid wearing tight clothing and high heels for long periods of time.
  • Use sunscreen on your face every day and avoid excessive exposure to heat.

 

What are the modern treatment methods?

Spider veins can be eliminated using safe, non-invasive dermatological techniques:

  • Laser Treatment for Spider Veins (Exclusively / Vascular Lasers): This is the most recommended method for treating spider veins on the face and very fine red spider veins on the legs. The laser light is selectively absorbed by the hemoglobin in the blood within the vessel, causing it to be thermally sealed (photocoagulation) and gradually absorbed by the body, without damaging the skin.
  • Sclerotherapy: The treatment of choice for medium-sized and larger blue/purple spider veins of the lower extremities. It involves injecting a special sclerosing agent directly into the vein using an extremely fine needle, causing it to collapse and disappear permanently.

 

Is the treatment painful?

No, the treatments are completely tolerable. During laser treatment, patients feel only a slight tingling or warmth, which is minimized by the use of special cooling mechanisms. In sclerotherapy, the injections are virtually painless due to the very small size of the needle.

 

How many sessions are required, and when do the results become apparent?

The number of sessions depends on the extent, size, and depth of the veins. Typically, 2 to 4 sessions are required, spaced 4–6 weeks apart. The veins begin to fade gradually in the weeks following each session.

 

Is there a recovery period?

No time off from daily activities is required. After treatment, mild redness, slight swelling, or small bruises may occur, which subside within a few days. For lower-limb treatments, the use of graduated compression stockings may be recommended for a few days.
